What is eyelid surgery?

Eyelid surgery corrects puffiness and sagging around the eyes that occurs due to aging, genetics or other factors. By tightening the skin around the eyes and removing excess skin and/or fat deposits, eyelid surgery creates a smooth, more youthful appearance.

How is eyelid surgery (blepharoplasty) performed?

Eyelid surgery can involve the upper eyelids, lower eyelids, or both eyelids depending upon the goals of the surgery and recommendations of your surgeon:

Upper eyelid surgery involves the placement of incisions in the natural crease of the upper lid, making them well hidden when the eyes are open. The excess skin and protruding fat are removed and the incision is closed. An entire upper eyelid lift can be completed in approximately 45 minutes to one hour. Detroit Plastic Surgery Center surgeons may combine upper eyelid surgery with an eyebrow lift to achieve optimal rejuvenation results if the eyebrows are too low. Lower eyelid lift can correct conditions with an incision just below the lower lash line. Excess skin in the lower eyelid is removed through this incision. Your cosmetic surgeon may also use an incision hidden inside the lower eyelid, or a trans-conjunctival incision, as an alternative technique to correct lower eyelid conditions and redistribute or remove excess fat.

If the trans- conjunctival approach is used, there is no visible incision. If the lower eyelids are lax, as they are in many middle-aged individuals, your surgeon may recommend tightening the lower lids in addition to removing skin and/or fat (canthopexy or canthoplasty).

Eyelid surgery usually takes about two hours to complete if both upper and lower eyelids are completed at the same time. Typically, a local anesthesia is combined with conscious sedation (also known as “twilight sleep”).

Minimal eyelid incision: The surgeon makes incisions following the natural lines of your eyelids whenever possible. Through these incisions, the surgeon removes excess fat, skin and muscle to reduce puffiness. The incisions are then closed with very small sutures.

Transconjunctival: The surgeon makes an incision inside the eyelid so that no incision is visible after surgery. Through this incision, the surgeon removes fat to correct bags in the lower eyelids. Fine lines in the skin can be softened with a special laser when using this method.

Eyelid surgeries are most commonly performed to achieve the following results:

Step 1 Consultation

A thorough consultation is an important first step when deciding if eyelid surgery is the right option for you. One of our board certified surgeons will discuss your desired results, examine you and provide an honest, professional opinion on how best to achieve those results. We also make certain that you are a good candidate for the procedure, taking into account your age, health, medical background and other factors that could affect the safety and effectiveness of your surgery. All of our consultations are conducted with the goal of making sure our patients feel comfortable, confident and informed about the eyelid surgery process.

Step 2 Preparation

Well before your procedure, let Detroit Plastic Surgery know of any medications you take. Three weeks prior and three weeks after, we strongly suggest limiting your alcohol and tobacco intake. Two weeks prior and two weeks after, you should not take any medication that may thin your blood such as aspirin or ibuprofen products.

The evening before your surgery, refrain from eating or drinking anything including water, and do not smoke. If you develop a cold, sore throat, fever or any other symptoms of illness before your surgery, let Detroit Plastic Surgery know right away.

The day of your procedure, refrain from wearing makeup, contact lenses, hairpins, jewelry, lotion or perfume. We also advise wearing comfortable clothing. Prepare to have a responsible adult provide you with transportation after your procedure and make arrangements for any care or assistance that you may need 24 hours after surgery.

Step 3 Surgery

Before your procedure, your surgeon will answer any questions that may have come up since your consultation. You will also have the opportunity to meet the Detroit Plastic Surgery staff assisting your surgeon during your procedure. Every effort is made to make certain that all of your concerns are addressed and you are as comfortable as possible.

When it is time to begin, you will be taken to our state-of-the-art surgical suite and comfortably fall asleep under general anesthesia. Eyelid surgeries typically take one to two hours to perform. If eyelid surgery is performed in combination with other procedures, surgery time will be longer.

Step 4 Recovery

We recommend no heavy lifting and avoiding direct sunlight for several weeks following your surgery. Antibiotic eyedrops or ointment may be prescribed if necessary. If you wear contact lenses or eye glasses, be prepared to not wear them for a couple of weeks following your procedure. We also strongly advise not wearing any makeup for seven to ten days after surgery. In just a few weeks, you can resume your daily activities, but it is important to remember that everyone recovers differently, and you may need more or less time before you feel 100% again. A few days after your surgery, your Detroit Plastic Surgery doctor will follow-up to see how you are healing and prescribe any necessary medication.

Step 5 Results

While results vary based on the individual, eyelid surgery patients typically enjoy a fresh, energetic appearance thanks to alert, youthful looking eyes. During your consultation, your doctor will thoroughly explain what results to expect.

Eyelid surgery is a common procedure for both men and women who want to reduce the bags and wrinkles from around their eyes and create a more youthful and alert appearance. All eyelid surgery candidates must be in good health and have no pre-existing medical conditions that could create complications.

Q: What causes the area around the eyes to age?

Q: What are the risks of eyelid surgery?

As with any surgical procedure, there is a risk of complication related to infection. Some people may wake up from surgery feeling groggy and experience an upset stomach, side effects of the anesthesia that are very temporary. We are happy to explain in detail any of the risks involved prior to your surgery. We carefully review your complete medical history and all medications you are on to prevent any complications.

Other risks associated with eyelid surgery may include:

  • Unfavorable scarring
  • Temporarily blurred or impaired vision

  • Numbness or other changes in skin sensation

Q: Can I have additional procedures in combination with my eyelid surgery to enhance results?
